Notice of Contract - Professional Sub-consultant - Corporation (Louisiana)

This Notice of Contract - Professional Subconsultant form is for use by a corporate Professional Subconsultant to provide notice to an owner of a contract, within 30 days after the date that the subconsultant enters into a written contract of employment. This notice include the name and address of the Employer (Prime Professional), the date of the employment/contract, a general description of the work undertaken, a description of the property where the construction is to be performed, the price of the work to be performed or method by which the price will be calculated, and the date payment for the work is to be made.
