Revocation of Health Care Directive (Oklahoma)

This form provides for partial or total revocation of the Advance Directive for Health Care provided in Form OK-P021, which allows you to express specific instructions for life sustaining procedures or treatment, appoint a health care agent, and make anatomical gifts. You may revoke in whole or in part an advance directive at any time and in any manner such as through this form, without regard to your mental or physical condition. (more...)
